Four sources, and they do not look alike

Water reaching a ceiling comes from a roof or flashing failure, a supply or drain line in the floor above, an attic air conditioning unit or its condensate line, or a bathroom on the upper level. Each has a signature. A roof leak worsens during rain and quiets between storms. A supply leak runs constantly. A drain leak appears when a fixture upstairs is used. An air conditioning condensate problem shows up in cooling season, often as a stain directly under an attic air handler or along the drain line's path. The diagnosis is not academic, because a ceiling repaired without the source stopped is a repair scheduled to fail.

The mechanical side has its own local rules

Safety before diagnosis

A ceiling that is sagging, bulging, or has water visibly pooling behind it gets the room below it cleared of people.
